More people are supported or more activity is delivered, but the system constraint remains and flow does not improve.
Funding the intervention displaces investment in the condition that would have made the greater difference.
A service can be useful, fully staffed and fully used without delivering the system outcome for which it was commissioned.
Test the chain before the investment becomes difficult to change.
Identify the missing condition early enough to fund it, redesign the intervention or redirect the investment.
